We can help make it easier for you and your patients to get the resources you both may need. Janssen CarePath helps verify insurance coverage for your patients, provides reimbursement information, helps find financial assistance options for eligible patients, and provides ongoing support to help patients start and stay on DARZALEX® as prescribed.

Affordability supportto help your patients start and stay on treatment you prescribe

Janssen CarePath can help you find out what affordability assistance may be available for your patients taking Janssen medicines:

  • Support for patients using commercial or private insurance
    • Janssen CarePath Savings Program allows eligible patients to pay $5 for each infusion for DARZALEX® (daratumumab) with a $20,000 maximum program benefit per calendar year
      • Patients using Medicare, Medicaid, or other government-funded programs to pay for their medications are not eligible to participate. For medication costs only; program does not cover cost to give patients their treatment. See full eligibility requirements at Darzalex.JanssenCarePathSavings.com.
      • JanssenCarePathPortal.com allows you to enroll patients in Savings Programs, upload EOB forms from your patients’ insurance providers, view patients' available benefit and transactions, and receive timely alerts and program updates.
  • Support for patients using government insurance or patients without insurance coverage:
    • Janssen CarePath can provide information about independent resources that may be able to help your patients with their out-of-pocket medication costs, including State-Sponsored Programs, Medicare Savings Program, Medicare Part D Extra Help—Low-Income Subsidy, and Independent Foundations.

We understand that navigating payer processes and finding cost support options may seem complicated at times. That’s why we’ve created an enhanced Provider Portal experience. The Janssen CarePath Provider Portal gives you 24-hour online access to:

  • Request benefits investigations
  • Review the status of benefits investigations
  • Enroll your eligible, commercially insured patients in the Janssen CarePath Savings Program
  • View savings program transactions for enrolled patients
  • Receive notifications when new information is available or action is required on the Portal

Create a Provider Portal account at JanssenCarePathPortal.com

  • Complete required information, include your practice locations, add administrators and staff to your site, and set communication preferences

Complete the Business Associate Agreement (BAA) for your practice (one time only)

  • The completed BAA allows you to request verification of patients’ insurance benefits and enroll eligible patients in the Janssen CarePath Savings Program without requiring individual patient authorization
    • You can execute the BAA within the Portal and receive immediate verification and access to the Portal, or
    • You can download the BAA at JanssenCarePath.com and upload the signed document via the Portal or fax to the number provided

OR

Secure patient authorization (for each patient)

  • Invite each patient to create an online account at MyJanssenCarePath.com to secure patient authorization, or
  • You can download the Patient Authorization Form at JanssenCarePath.com and upload the signed document via the Portal or fax to the number provided

Now you are ready to submit a benefits investigation on the Portal.

Cost Support

Your patients may be eligible for programs that can help them afford their treatment with DARZALEX®.

Affordability Options for DARZALEX®

If your patient has commercial or private health insurance

Janssen CarePath Savings Program for DARZALEX®

Your eligible patients will pay $5 for each infusion, with a $20,000 maximum program benefit each calendar year. The Janssen CarePath Savings Program provides financial assistance to help patients with their out-of-pocket costs on their DARZALEX® medication, depending on the patient's health plan, savings may apply toward the patient's deductible, co-payment, or coinsurance costs. Eligible patients must use commercial or private health insurance that covers a portion of their medication costs for DARZALEX®. The program does not cover cost to give patients their treatment. Patients using Medicare, Medicaid, or other federally funded programs to pay for DARZALEX® medication are not eligible to participate.

If your patient has Medicare

Janssen CarePath can refer patients to the Medicare Savings Program to discuss eligibility and program benefits. To learn more about eligibility and how the Medicare Savings Program can help patients pay for Medicare Part B products, visit medicare.gov/contacts/#resources/msps and select the appropriate state.

If your patient has government-funded insurance such as Medicare, Medicaid, or need supplemental assistance with paying for your medication

Independent Foundation support may be available:

Independent co-pay assistance foundations have their own rules for eligibility. We have no control over these independent foundations and can only refer your patients to a foundation that supports their disease state. We do not endorse any particular foundation. The foundations on this list are not the only ones that might be able to help your patients.

If your patient has no health insurance or no coverage
